EMERGENCY CONSENT
I grant my permission for medical treatment and procedures, as deemed necessary, to be administered to my child while participating in the Children’s Ministries at Pine Forest Baptist Church. In case of an emergency and if I cannot be reached, I hereby authorize the Children’s Ministries staff to administered needed first aid or to seek medical attention for my child. I hereby give consent for the medical care providers and local hospital listed under “Medical Providers” to be called when I cannot be reached.
PARENT INVOLVEMENT AND REGISTRATION
For the success of your child’s spiritual life at church:
Often a school teacher or sports coach will ask for your help. They know how valuable it is for you to be involved in your child’s life and those things that are so important to him/her. Equally valuable is your involvement in your child’s spiritual growth and relationship at church. Children love to see parents helping at church. Surely, the success of your child’s Christian education at Pine Forest Baptist is dependent upon your active involvement. Please be prayerfully considering where you can help in the Children’s Ministries at Pine Forest Baptist Church. It may be anything from assisting in classroom activities, gathering supplies, providing snacks, helping with big events, or chaperoning on trip. Please contact Kelly Smith for more information on the different needs in this area.
I hereby consent to have my son/daughter participate in children’s activities at Pine Forest Baptist Church and to be transported by church vehicles when necessary. I also consent to my child’s photograph being used on PFBC’s media.
